Quercetin interacts with whole categories of medication. Apalutamide falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down apalutamide (Erleada), and quercetin can nudge that enzyme. The tricky part is that the research points in both directions. In some studies quercetin slows CYP3A4 down (which could let a drug build up a bit), and in one study it actually sped the enzyme up (which could lower drug levels). So we honestly can't say for sure which way apalutamide would shift.

The good news is that this is mostly lab and animal data, and apalutamide itself is an active drug, not a prodrug that needs activating. Food amounts of quercetin from fruits and veggies are very unlikely to matter. Concentrated supplements are a different story, so before adding one, check with your oncology team or pharmacist so your cancer treatment stays on track.

Apalutamide also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the basic idea with Erleada (apalutamide) and quercetin. Your body uses a liver enzyme called CYP2C8 to help break apalutamide down. In test-tube studies, quercetin can slow that enzyme down a bit. In theory, if it slows clearance of apalutamide, more of the drug could hang around, which might mean stronger effects and a higher chance of side effects like fatigue, joint pain, rash, high blood pressure, or, less often, falls or seizures.

That said, I want to keep this in perspective. This is theoretical and based only on lab data, and a human study with a similar drug showed no real effect. Quercetin in everyday foods is very unlikely to matter. With concentrated supplements, just run it by your oncologist or pharmacist first.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
