Moringa and Estradiol, Progesterone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Moringa interacts with whole categories of medication. Estradiol, Progesterone falls into 3 of those categories, so it can interact with Moringa in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 3

Why this applies to Estradiol, Progesterone

Estradiol, Progesterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your Bijuva combines estradiol and progesterone, and your body relies on a liver enzyme called CYP3A4 to help break both hormones down. Moringa might nudge this enzyme one way or the other, but honestly the science here is thin. Lab and test-tube studies disagree: some hint moringa slows CYP3A4 (which could let hormone levels creep up), while others suggest it speeds the enzyme up (which could lower hormone levels).

In practice, a real human study found moringa didn't meaningfully change a similar CYP3A4 drug. So this is mostly theoretical. Food amounts in cooking are very unlikely to matter; concentrated daily supplements are the bigger question mark. If you'd notice things like spotting, breast tenderness, or mood shifts, just loop in your pharmacist or doctor before combining them.

P-GLYCOPROTEIN SUBSTRATES

2 of 3

Estradiol, Progesterone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the simple version for Bijuva (estradiol and progesterone). Lab studies suggest moringa leaf extract can block a transporter called P-glycoprotein, which helps move certain medicines around and out of the body. In theory, slowing that pump could let a little more of your hormone medicine hang around, which might mean a touch more of the usual effects, like breast tenderness, bloating, nausea, headaches, or mood changes.

The honest part: this is based only on test-tube research, and it has never actually been reported in real people. So I wouldn't lose sleep over it, especially with normal food amounts of moringa. If you take concentrated moringa capsules daily alongside Bijuva, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

3 of 3

Estradiol, Progesterone also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, moringa might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that moringa extract induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Bijuva (estradiol and progesterone). Your liver uses an enzyme called CYP1A2 to help break down estradiol. In lab studies (not in people), moringa seemed to speed that enzyme up. If that held true in the body, it could clear estradiol a little faster and make the hormone therapy work slightly less well. That might show up as the symptoms you're treating creeping back, like hot flashes, night sweats, or sleep trouble.

The reassuring part: this is theoretical and only seen in test-tube research, and it's rated minor. Food amounts of moringa are unlikely to matter. If you want to use concentrated moringa supplements regularly, just run it by your pharmacist or doctor first so they can keep an eye on how you're feeling.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Moringa

What is Moringa?

  • Nutritional supplement
  • Blood sugar support
  • Cholesterol support
  • Inflammation
  • Skin conditions

Moringa is a nutrient-rich plant whose leaves are widely used as a food and supplement, especially in parts of the world where malnutrition is common. Early research hints at possible benefits for blood sugar, cholesterol, and inflammation, but the human evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using moringa, especially if you are pregnant, breastfeeding, or taking medications.
