CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Chlorpheniramine, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Let me put your mind at ease here. This combo cold medicine has three ingredients, and the one that touches the CYP3A4 pathway is chlorpheniramine (the antihistamine). The concern on file is only theoretical, coming from lab and animal studies, not real people. In fact, a human study found moringa didn't change levels of another CYP3A4 drug. The lab data is also mixed, with some saying moringa speeds the enzyme up and some saying it slows it down.
If moringa did slow that enzyme, chlorpheniramine could linger a bit longer, meaning more drowsiness or dry mouth. Honestly, though, this is unlikely to cause real trouble. Food amounts of moringa shouldn't matter. If you take concentrated supplements regularly, just mention it to your pharmacist.

Here's the simple version. This combo product holds three medicines: acetaminophen for pain and fever, chlorpheniramine for sniffles and sneezing, and pseudoephedrine to clear a stuffy nose. Lab studies suggest moringa may slightly speed up an enzyme called CYP1A2, which helps your body break down certain drugs. If that happens, a medicine cleared by that enzyme could be removed a bit faster, possibly making it feel less effective.
The good news: this is rated minor, and the evidence comes only from test-tube and animal work, not from people. The amounts in food and typical supplements are unlikely to cause a noticeable change. Still, if you take moringa regularly, it's worth a quick chat with your pharmacist or doctor before mixing the two.
