Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the situation with moringa and Calquence (acalabrutinib). Your body relies on a liver enzyme called CYP3A4 to break down acalabrutinib. In test-tube studies, moringa has gone both ways: some lab work suggests it could block (inhibit) this enzyme, while other work suggests it could speed it up (induce it). Acalabrutinib is an active drug, not a prodrug, so if moringa slows CYP3A4 the drug could build up and raise the risk of side effects like bruising, bleeding, headaches, or infections. If moringa speeds the enzyme up instead, the drug could be cleared too fast and work less well against your cancer.

The honest truth is this is theoretical, based mostly on lab data, and one human study showed no real effect with another drug. Still, because acalabrutinib treats cancer, please run any moringa supplement past your oncologist or pharmacist first. Occasional culinary amounts are far less concerning than concentrated daily supplements.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist: acalabrutinib (Calquence) is moved around your body partly by a transporter called P-glycoprotein, which acts like a doorway helping the drug exit. Lab studies suggest moringa leaf extract may block some of that transporter activity. In theory, if that doorway is partly closed, more acalabrutinib could stay in your system, which might mean stronger effects and a higher chance of side effects like bruising or bleeding, headaches, low blood counts, fatigue, or infections.

I want to be honest about the evidence, though. This is based only on test-tube work, and it has not been seen in real people. Acalabrutinib is also a cancer medicine where dosing matters a lot, so don't start or stop moringa on your own. Please run it by your oncologist or pharmacist first.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
