Clomipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with these two. Your body partly relies on a liver enzyme called CYP1A2 to clear clomipramine out of your system. In test-tube studies, neem leaf extract seems to slow that same enzyme down. If that happened in a real person, clomipramine could clear more slowly and build up a bit, which can mean stronger side effects like dry mouth, constipation, drowsiness, dizziness, a faster heartbeat, or trouble urinating.
I want to be honest about the strength of this, though. This is only lab-based, and it has not actually been reported in people. So I wouldn't panic. Still, since clomipramine is sensitive to dosing, please run any neem supplement past your pharmacist or doctor before combining them.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses an enzyme called CYP3A4 to help break down clomipramine. In test-tube studies, neem leaf extract appears to slow that enzyme down. If that happened in real life, your clomipramine could clear out more slowly and build up a little, which might mean stronger effects and more of the usual tricyclic side effects (dry mouth, constipation, drowsiness, blurry vision, dizziness, or a faster heartbeat).
The important caveat: this is only seen in lab dishes, not in actual people, so it's theoretical right now. Food amounts in things like neem toothpaste are unlikely to matter. Before adding a concentrated neem supplement, just run it by your pharmacist or doctor so they can keep an eye on how you feel.

Here's the idea behind this pairing. Your body uses several liver enzymes to clear clomipramine, and one of them is CYP2C9. In test-tube studies, neem leaf extract seems to slow that enzyme down. If that happened in real life, clomipramine could clear a bit more slowly and build up, which might nudge up its usual side effects like dry mouth, constipation, drowsiness, dizziness, blurred vision, or a faster heartbeat.
I want to be honest with you though: this is only a lab finding, and it hasn't been reported in actual people. Clomipramine is active on its own (not a prodrug), so any change would lean toward stronger effects, not weaker. Before adding neem, just check with your pharmacist or doctor so they can keep an eye on how you feel.
