Why this applies to Crizotinib

Crizotinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist. Crizotinib (Xalkori) is moved around your body partly by a transporter called P-glycoprotein, which acts like a little pump helping your cells push the drug out. Lab studies suggest neem leaf extract may slow that pump down. If that happened in people, more crizotinib could stay in your system, which could mean stronger effects and a higher chance of side effects like nausea, vision changes, swelling, fatigue, or liver and heart rhythm issues.

The important part: this is only a test-tube finding and hasn't been seen in real patients, so it stays theoretical. Still, with a cancer medicine like this, I wouldn't gamble. Please check with your oncologist or pharmacist before using any neem product, and don't change your crizotinib on your own.

Crizotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the idea worth knowing about. Your body uses an enzyme called CYP3A4 to break down crizotinib (Xalkori) and clear it out. In lab dish studies, neem leaf extract seems to slow that same enzyme down. If neem blocked it in real life, crizotinib could build up higher than intended, which might mean more of its known side effects, things like nausea, vomiting, diarrhea, vision changes, swelling, fatigue, or effects on heart rhythm and liver values.

The honest part: this is theoretical. It comes only from test-tube work and hasn't been seen in actual patients. Still, because crizotinib is a serious cancer drug with a tight safety window, I wouldn't add neem without first checking with your oncologist or pharmacist.

About Crizotinib

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) treats adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for ALK or ROS1 gene changes, as confirmed by an FDA-approved test.

Xalkori is also used for pediatric patients (age 1 and older) and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL), and for adult and pediatric patients (age 1 and older) with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
