Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection with Fiorinal (aspirin, butalbital, and caffeine). The caffeine part of this combo is cleared from your body mainly by a liver enzyme called CYP1A2. Lab studies suggest neem leaf extract may slow that enzyme down. If that happened in real life, caffeine could linger longer and build up a bit, which might leave you feeling jittery, restless, sped-up, with a racing heart or trouble sleeping.
I want to be honest about the strength of this, though. So far this is only seen in test-tube research, never reported in actual people, so it's more of a theoretical heads-up than a proven problem. The aspirin and butalbital parts aren't really the concern here. If you want to use neem regularly, just run it by your pharmacist or doctor first.

Here's the gist with Fiorinal (aspirin, butalbital, and caffeine). In lab dishes, neem leaf extract appears to slow down a liver enzyme called CYP3A4. The butalbital part of your Fiorinal leans on that enzyme to be cleared from your body. So in theory, if neem blocks that pathway, butalbital could hang around longer and build up, which might mean more drowsiness, dizziness, or that foggy, overly sedated feeling.
I want to keep this in perspective. This has only been seen in test tubes, never reported in real people, so the actual risk is uncertain and likely small. Still, since butalbital can already make you sleepy, it's worth a quick chat with your pharmacist or doctor before pairing it with neem supplements.
