Aspirin, Butalbital, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
The piece of Fiorinal that matters here is the aspirin. Aspirin already thins the blood by making your platelets less sticky, so they don't clump together as easily. Danshen seems to do something similar, slowing down clotting on its own. Put the two together and you may get an additive effect, meaning a higher chance of bleeding or bruising than you'd expect from the aspirin alone.
Real-world things to watch for would be easy bruising, nosebleeds, bleeding gums, or dark or tarry stools. To be fair, this concern is mostly theoretical and based on animal and anecdotal reports, not strong human studies. Still, since it's rated a major interaction, please check with your pharmacist or doctor before pairing them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the part worth knowing about this combo (Fiorinal). The caffeine in it is cleared by a liver enzyme called CYP1A2. In test tube studies, Danshen seems to slow that enzyme down, which could in theory let caffeine build up a bit. If that happened, you might feel more of caffeine's usual effects, like jitteriness, a racing heart, trouble sleeping, or feeling on edge.
The honest takeaway: this is very weak evidence. It comes only from lab work, and when researchers tested a similar CYP1A2 drug (theophylline) in real people, Danshen didn't actually cause a meaningful change. Danshen's bigger concern with Fiorinal is actually the aspirin, since both can affect blood clotting. Please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

If you take Fiorinal (aspirin, butalbital, and caffeine) for tension headaches, here's the gist with Danshen. The butalbital piece is partly broken down by a liver and gut enzyme called CYP3A4. In short studies, taking Danshen for about 10 to 14 days seemed to speed that enzyme up, which could clear butalbital a little faster and make your headache combo feel weaker than usual. Interestingly, a single dose of Danshen did the opposite and could raise levels, leaving you groggier or more sedated.
There's another wrinkle: Danshen may thin the blood, and aspirin already does too, so combining them could raise your bleeding or bruising risk. The evidence here is early and modest, not rock solid. Please don't change anything on your own. Just run it by your pharmacist or doctor first.

ASPIRIN
4 of 4Research in healthy adult males shows that taking a combination of danshen and kudzu with aspirin increases plasma aspirin area under the curve by approximately 3.4-fold. Animal research also shows that taking a combination of danshen and kudzu (danshen-gegen formula) with aspirin increases maximal blood levels of aspirin and salicylic acid by approximately 4-fold and 3.7-fold, respectively, without impacting blood loss. Taking danshen increases the antiplatelet activity of aspirin and might increase the side effects of aspirin.

Here's the thing to watch with Fiorinal: it contains aspirin, which already thins the blood a bit by making your platelets less sticky. Danshen seems to do two things on top of that. It can push aspirin levels in your blood higher, and it has its own mild blood-thinning effect. In one human study, combining danshen (with kudzu) raised aspirin exposure by roughly three to four times.
What that means in real life is a higher chance of bleeding-type problems, like easy bruising, nosebleeds, bleeding gums, or stomach upset and even stomach bleeding. The butalbital and caffeine parts aren't the concern here, it's the aspirin. Please don't start or stop anything on your own. Just check with your pharmacist or doctor first so they can weigh in.
