Grape and Crizotinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Crizotinib

Crizotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with grape seed extract and your Xalkori (crizotinib). Your body relies on a liver enzyme called CYP3A4 to break down crizotinib. Concentrated grape seed extract may tinker with that enzyme, but honestly, the research pulls in two directions. Lab (test tube) studies hint it could slow the enzyme, which would let crizotinib build up and possibly mean more side effects like nausea, vision changes, swelling, fatigue, or effects on your heart rhythm. Animal studies suggest the opposite, that it could speed up clearance and weaken the drug.

The key point: this has never actually been seen in people, and it's based on early lab and animal data only. Eating grapes is fine. Before starting a grape seed supplement, please check with your oncologist or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Crizotinib page

“Grapefruit tops my worry list for crizotinib (Xalkori), a targeted pill for ALK-positive lung cancer. It blocks the enzyme (CYP3A4) that clears crizotinib, letting levels climb and side effects with them, and it is one of eight major-rated entries among the 157 supplements and herbs on file for this drug.”
