Why this applies to Aliskiren
Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with grape seed extract and your blood pressure medicine, aliskiren (Tekturna). Aliskiren is partly handled by a liver enzyme called CYP3A4, and grape compounds may nudge that enzyme one way or the other. The catch is that the lab and animal studies disagree: some suggest grape seed extract slows this enzyme (which could let more aliskiren build up, possibly causing dizziness, lightheadedness, or blood pressure dropping too low), while animal data suggest it might speed the enzyme up (which could weaken the medicine and let your pressure creep back up).
The important thing is that none of this has actually been seen in people, so it's mostly theoretical. Eating grapes in normal amounts is very unlikely to matter. Concentrated supplements are the real question mark, so check with your pharmacist or doctor before adding one.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
