Yerba Mate and Aliskiren

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yerba Mate interacts with.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Tekturna (aliskiren) and yerba mate. Aliskiren is partly handled by a liver enzyme called CYP3A4, and lab studies (in test tubes, not people) suggest yerba mate may slow that enzyme down. In theory, that could let a little more aliskiren build up in your system, nudging its blood-pressure-lowering effect a bit stronger. If that happened, you might notice things like dizziness, lightheadedness when standing, or feeling unusually tired.

I want to be honest though: this is theoretical and based only on lab work, not real-world reports, so it's rated minor. A casual cup of mate is unlikely to cause trouble. If you drink it daily or take concentrated mate supplements, just mention it to your pharmacist or doctor so they can keep an eye on your blood pressure.

From our pharmacist’s overview on the Aliskiren page

“Aliskiren (Tekturna) has one job, keeping blood pressure down, and most of this list pulls on that same rope. Of the 227 supplements with an interaction on file, 119 have blood-pressure effects of their own, stacking with the drug until pressure drops too far (fish oil is a common one) or pushing back against it.”

About Yerba Mate

What is Yerba Mate?

Yerba Mate
  • Energy and alertness
  • Weight loss
  • Mental focus
  • Digestive support
  • Antioxidant support

Yerba mate is a caffeine-containing herbal beverage from South America that is widely enjoyed for its stimulating, coffee-like effects. While it is rich in antioxidants and is being studied for weight, blood sugar, and cholesterol, strong human evidence for most health claims is limited. Drinking very hot mate over many years has been linked to higher risk of certain cancers, so enjoy it warm and in moderation.

About Aliskiren

What Is Aliskiren Used For?

Brand name: Tekturna

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
