Darunavir, Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the situation with your HIV combo, darunavir with ritonavir. In test-tube studies, neem leaf extract seems to slow down a cellular pump called P-glycoprotein, which helps move drugs like these out of your body. If that pump is blocked, in theory more of the medication could hang around, which might mean a bit more of the usual side effects such as nausea, diarrhea, stomach upset, or headache.

The key word here is theoretical. This has only been seen in lab dishes, not in real people, so I wouldn't lose sleep over it. Still, since staying steady on HIV therapy matters a lot, please loop in your pharmacist or doctor before adding a neem supplement so we can keep an eye on things.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the situation with your HIV combo, darunavir with ritonavir. Both of these are cleared by a liver enzyme called CYP3A4. In test-tube studies, neem leaf extract appears to slow that enzyme down. If that happened in your body, these medicines could build up higher than expected, which might mean more side effects like nausea, diarrhea, headache, or changes in blood sugar and cholesterol.

Honestly, this is a theoretical concern only. It's been seen in the lab, not in real people, so I don't want to alarm you. Ritonavir itself is actually a strong CYP3A4 blocker (that's part of how it works), so a plant extract adding to that is hard to predict. Because keeping your HIV treatment steady matters a lot, please check with your pharmacist or doctor before adding neem.
