Erythromycin Gluceptate falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for erythromycin (the gluceptate is just an injectable form). Your body uses a tiny cellular pump called P-glycoprotein to help move erythromycin out of cells and clear it. Lab and animal studies suggest neem leaf extract may slow that pump down. If that happened in people, a little more erythromycin could linger in your system, which could nudge up its usual side effects like nausea, stomach cramping, diarrhea, or in rare cases heart rhythm changes (a longer QT interval).
The honest part: this is theoretical and has only been seen in test tubes, never reported in actual patients. So I wouldn't lose sleep over it. Still, since erythromycin is usually given in a hospital setting, let your doctor or pharmacist know if you take neem so they can keep an eye out.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Here's the gist for erythromycin: your liver uses an enzyme called CYP3A4 to help break it down. In lab tests (test tubes, not people), neem leaf extract seems to slow that same enzyme down. If that held true in the body, erythromycin could clear out a bit slower and build up, which might nudge its effects and side effects upward, things like nausea, stomach cramps, or diarrhea, and rarely heart rhythm changes (a longer QT interval).
I want to be honest with you though: this is theoretical. It has not been reported in real patients, and we only have early lab data. Erythromycin isn't a prodrug, so it works as-is rather than needing activation. Food-amounts of neem are unlikely to matter, but concentrated supplements are a different story. Please run it by your pharmacist or doctor first.
