These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Neem interacts with whole categories of medication. Ombitasvir, Paritaprevir Ritonavir falls into 2 of those categories, so it can interact with Neem in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
P-GLYCOPROTEIN SUBSTRATES
1 of 2Why this applies to Ombitasvir, Paritaprevir Ritonavir
Ombitasvir, Paritaprevir Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Technivie (ombitasvir, paritaprevir, and ritonavir). One of the ways your body moves this medication around uses a transporter called P-glycoprotein. In lab dish studies, neem leaf extract seemed to slow that transporter down. In theory, that could let a little more of the drug build up in your system, which might nudge up side effects like tiredness, nausea, itching, or trouble sleeping.
I want to be honest about how thin this evidence is. This comes only from test-tube research, and it has never actually been reported in real people taking these medicines. So I wouldn't lose sleep over it, but since you're treating hepatitis C and want this regimen working just right, run any neem product past your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Ombitasvir, Paritaprevir Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Technivie (ombitasvir, paritaprevir, and ritonavir): your body uses an enzyme called CYP3A4 to help process parts of this combo, especially the paritaprevir. In lab studies only, neem leaf extract seems to slow that enzyme down. In theory, that could let levels of the medicine creep up, which might mean more side effects like nausea, tiredness, itching, or trouble sleeping.
I want to be honest though: this is based on test-tube work, not real people, so it's a theoretical concern rather than something we've actually seen happen. Still, since you're treating hepatitis C and protecting your liver matters, please run any neem product past your pharmacist or doctor before mixing the two. Don't change your treatment on your own.

From our pharmacist’s overview on the Ombitasvir, Paritaprevir Ritonavir page
“Technivie (ombitasvir, paritaprevir, and ritonavir) treats hepatitis C, and because a cure depends on holding steady drug levels for the whole course, this list warrants genuine care. Most of the 154 ingredients on file touch the CYP3A4 enzyme pathway, meaning they can push the drug's blood levels up or down.”

What is Neem?
- Skin conditions
- Dental and gum health
- Antimicrobial and antiparasitic
- Digestive upset
- Blood sugar support
- Insect repellent/pesticide
Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
About Ombitasvir, Paritaprevir, and Ritonavir
What Is Ombitasvir, Paritaprevir, and Ritonavir Used For?
Brand name: Technivie
-
The combination of ombitasvir, paritaprevir, and ritonavir is usually used in combination with ribavirin (Copegus, Rebetol), but sometimes it is used alone to treat a certain type of chronic (long-term) hepatitis C infection (swelling of the liver caused by a virus). Ombitasvir is a hepatitis C virus (HCV) NS5A inhibitor. It works by stopping the virus that causes hepatitis C from spreading inside the body. Paritaprevir is a protease inhibitor. It works by decreasing the amount of HCV in the body. Ritonavir is a protease inhibitor. It helps to increase the amount of paritaprevir in the body so that the medication will have a greater effect. It is not known if ombitasvir, paritaprevir, or ritonavir prevent the spread of hepatitis C to other people.
