Naproxen, Pseudoephedrine brand name: Aleve Cold and Sinus.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Neem interacts with whole categories of medication. Naproxen, Pseudoephedrine falls into 2 of those categories, so it can interact with Neem in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Naproxen, Pseudoephedrine
Naproxen, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Aleve Cold and Sinus (naproxen plus pseudoephedrine) and neem. In lab dishes, neem leaf extract seems to slow down a liver enzyme called CYP1A2. If that same slowdown happened in people, this medication could clear a bit more slowly and build up, which in theory could nudge its effects and side effects upward. For naproxen that might mean more stomach upset, heartburn, or fluid retention, and for the pseudoephedrine piece it could mean more jitteriness, a faster heartbeat, or higher blood pressure.
That said, this is only a test-tube finding and has never been reported in real patients, so I wouldn't lose sleep over it. If you take neem regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 2Naproxen, Pseudoephedrine also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Aleve Cold and Sinus. The naproxen part of this combo is cleared by a liver enzyme called CYP2C9. In test-tube studies, neem leaf extract seems to slow that enzyme down. If it did the same thing in your body, naproxen could hang around longer and build up a bit, which might mean more of its usual side effects like stomach upset, heartburn, or a higher risk of stomach irritation or bleeding.
I want to be honest with you though: this is only seen in lab studies, not in real people, so it's more of a theoretical caution than a proven problem. The pseudoephedrine side isn't really involved here. If you take neem regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on you.

From our pharmacist’s overview on the Naproxen, Pseudoephedrine page
“Aleve Cold and Sinus pairs naproxen, an anti-inflammatory painkiller, with pseudoephedrine, a decongestant that acts as a mild stimulant, and that combination is why its supplement list runs to 334 entries and deserves real vigilance. The two biggest themes mirror the two drugs.”

About Neem
What is Neem?
- Skin conditions
- Dental and gum health
- Antimicrobial and antiparasitic
- Digestive upset
- Blood sugar support
- Insect repellent/pesticide
Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
