Avandamet combines two diabetes medicines, metformin and rosiglitazone, that both work to lower your blood sugar. The thing to know here is that neem may lower blood sugar on its own. Stack that on top of your medication and the two can add up, pushing your blood sugar lower than you intended.

If that happens, you might feel shaky, sweaty, dizzy, irritable, or suddenly very hungry. Keep in mind the proof here is still pretty thin (mostly lab and animal work, plus a few small human studies), so this isn't a guaranteed problem. Still, if you'd like to try neem, it's worth a quick chat with your pharmacist or doctor first, and checking your blood sugar a little more closely for a while is a smart move.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.

Avandamet combines two diabetes medicines. The rosiglitazone part is partly cleared by a liver enzyme called CYP2C9, and lab studies suggest neem leaf extract may slow that enzyme down. If that happened in real life, rosiglitazone could build up a bit and push its effects a little harder. That might mean slightly more blood sugar lowering, plus things rosiglitazone is known for, like fluid retention, swelling in the ankles, or weight gain. The metformin half isn't handled by this enzyme, so it's not the concern here.

Here's the reassuring part: this is only a test-tube finding and hasn't been seen in actual patients. Still, since blood sugar control matters, mention any neem supplement to your pharmacist or doctor before adding it.

This combination pill pairs metformin with rosiglitazone, and it's the rosiglitazone half that matters here. Rosiglitazone is broken down by a liver enzyme called CYP2C8. In lab tests, neem leaf extract seemed to slow that enzyme down. If that happened in a real person, rosiglitazone could build up a bit more than expected, which might lead to side effects like fluid retention, swelling in the ankles, weight gain, or in some folks an extra drop in blood sugar (shakiness, sweating, dizziness).

The honest part: this is only a test-tube finding, never actually reported in people. So I wouldn't lose sleep over it, but I'd mention any neem supplement to your pharmacist or doctor before adding it.
