Avandamet combines metformin and rosiglitazone, two medicines that work to lower your blood sugar. The concern here isn't about acai changing how your body breaks down these drugs. Instead, acai itself may nudge blood sugar in either direction. Early research in healthy people found that acai sometimes raised and sometimes lowered fasting glucose, so the effect simply isn't predictable.

What this means for you: if you add a concentrated acai supplement on top of Avandamet, your readings could drift a bit higher or lower than usual. Eating acai as a food, like in a smoothie bowl, is very unlikely to cause trouble. Since this is theoretical, I wouldn't lose sleep over it, but if you start a supplement, check your sugars a little more closely and loop in your doctor or pharmacist.
