Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for your phenytoin (and the phenobarbital combined with it): your body uses a liver enzyme called CYP2C9 to help clear phenytoin out of your system. In lab dish studies, neem leaf extract seemed to slow that enzyme down. If that happened in a person, phenytoin could build up a bit higher than expected. Because phenytoin has a narrow safety window, even small increases can matter, possibly bringing on dizziness, unsteady walking, slurred speech, double vision, or unusual drowsiness.

I want to be honest, though: this is only theoretical, based on test-tube work, and it hasn't been seen in real patients. So it's not a reason to panic, just a reason to check in with your pharmacist or doctor before adding neem.

Here's the gist with this combo. In a lab dish, neem leaf extract slowed down an enzyme called CYP2C8, which is one of the pathways your body uses to clear medicines like phenytoin (Dilantin) and phenobarbital. In theory, if that pathway gets blocked, these drugs could build up a little and push you toward more side effects, things like drowsiness, dizziness, unsteady walking, slurred speech, or double vision.

The big caveat: this is only a test-tube finding and has never been reported in real people. Still, both of these are narrow-margin seizure medicines where small swings matter, so I wouldn't add a concentrated neem supplement without first checking with your pharmacist or doctor.
