Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on Entresto (sacubitril/valsartan) with neem. In a test tube, neem leaf extract slowed down a liver enzyme called CYP2C9. Valsartan, one half of Entresto, partly relies on that pathway. So in theory, neem could let a bit more valsartan linger in your body, nudging up its effects. This is an active drug, not a prodrug, so more drug means more action.
What might that look like? Lower blood pressure than expected, dizziness or lightheadedness (especially when standing), and possibly higher potassium. That said, this is only lab data and hasn't been seen in real people, so I wouldn't lose sleep over it. Still, please run any neem supplement past your pharmacist or doctor before adding it.

From our pharmacist’s overview on the Sacubitril, Valsartan page
“Entresto (sacubitril and valsartan) is a heart failure medicine, and heart failure patients tend to be heavy supplement users, reaching for fish oil, potassium, and herbal blood-pressure remedies. That instinct carries real risk here.”

About Neem
What is Neem?
- Skin conditions
- Dental and gum health
- Antimicrobial and antiparasitic
- Digestive upset
- Blood sugar support
- Insect repellent/pesticide
Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
About Valsartan and Sacubitril
What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).
It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
