Ensartinib falls under HEPATOTOXIC DRUGS — the group Kava interacts with.

Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the short version: this isn't a case of kava changing how well your ensartinib works. Instead, both can be tough on the liver, and stacking them may add up. Ensartinib (Ensacove) is a targeted cancer medicine for certain lung cancers, and one of its known issues is that it can raise liver enzymes and, in some people, cause liver injury. Kava has its own track record of rare but serious liver problems.

So putting them together is really about doubling up on liver stress. Keep in mind this concern is based on anecdotal reports, not strong studies, and most kava liver cases involved heavy, long-term use. Still, since your care team is already watching your liver labs on ensartinib, please tell your pharmacist or oncologist before adding kava so they can weigh in.

P-GLYCOPROTEIN SUBSTRATES

Ensartinib also falls under P-GLYCOPROTEIN SUBSTRATES.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the situation with kava and your cancer medicine Ensacove (ensartinib). Ensartinib is carried around your body partly by a little cellular pump called P-glycoprotein, or P-gp for short. In test-tube studies, kava seemed to slow that pump down. If that happened in real life, less ensartinib would get pushed out, so more could build up. In theory that could mean stronger side effects like nausea, rash, swelling, itching, or changes in your liver tests.

But the evidence here is thin and honestly conflicting. A human study using a standardized kava dose didn't change levels of another P-gp drug. On top of that, kava itself has been tied to rare liver injury, and ensartinib can already stress the liver. I'd loop in your oncology pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Ensartinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version for anyone taking Ensacove (ensartinib), a targeted cancer pill for certain ALK-positive lung cancers. Ensartinib is broken down by a liver enzyme called CYP3A4. The worry on paper is that kava might slow that enzyme down, which could let ensartinib build up and lead to more side effects like nausea, rash, or liver strain.

But the real-world evidence here is reassuring. A human study using a solid daily dose of kavalactones for a month showed no meaningful effect on CYP3A4. So while lab dishes hint at a problem, actual clinical data suggests this interaction is unlikely to matter. Still, kava itself can be hard on the liver, and so can ensartinib, so please loop in your oncology team or pharmacist before combining them.
