Diphenhydramine Hydrochloride, Naproxen Sodium falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to watch with Aleve PM: one of its two ingredients, diphenhydramine, is a sedating antihistamine (the same drug found in many nighttime sleep aids). Kava also has a calming, sedative effect on the brain. Put them together and those effects stack up, which can leave you more drowsy, foggy, and unsteady than you'd expect. We're not talking about the drug building up in your blood, just two sleepy-acting things working in the same direction.
That extra sedation matters most if you plan to drive, use machinery, or care for others, and the grogginess can be stronger in older adults. Expert anxiety treatment guidelines actually advise against mixing kava with other depressants. Please check with your pharmacist or doctor before combining them, and don't add alcohol on top.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Aleve PM combines two ingredients: naproxen (a pain and inflammation reliever) and diphenhydramine (the sedating antihistamine you'd recognize from Benadryl). The naproxen half is partly cleared by a liver enzyme called CYP2C9. In test-tube studies, kava blocks that enzyme, so in theory it could let naproxen build up a bit. With more naproxen around, you might be more prone to its usual side effects like stomach upset, heartburn, or, with longer use, stomach irritation or bleeding.
I want to be honest here: this is only seen in lab dishes so far, not in real people, so the concern is theoretical and likely small. The bigger practical issue is doubling up on drowsiness, since both kava and diphenhydramine are calming. Please check with your pharmacist before combining them.

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture on the metabolism side: Aleve PM combines diphenhydramine (the sleepy antihistamine) with naproxen (an anti-inflammatory pain reliever). The concern on file is whether kava blocks a liver enzyme called CYP1A2 that helps process certain drugs. The good news is that the better-quality human research shows kava doesn't meaningfully affect CYP1A2, so this particular interaction is rated minor and unlikely. Neither of these ingredients is a prodrug, so you don't need to worry about kava flipping how well they work.
My bigger, real-world caution is different: kava and diphenhydramine both calm the brain, so combining them can stack up drowsiness, grogginess, and dizziness. Both kava and naproxen can also stress the liver. Please check with your pharmacist before pairing them.
