Esomeprazole Strontium falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the gist for esomeprazole, which is the active part of this product (esomeprazole strontium). Your body uses a liver enzyme called CYP2C19 to help clear esomeprazole out. In lab dishes, kava appears to slow that same enzyme down. If that happened in people, more esomeprazole could hang around, which might mean a bit stronger effect and a slightly higher chance of side effects like headache, nausea, gas, or diarrhea.

The important caveat: this is only test-tube data and hasn't actually been shown in humans, so it's theoretical and likely minor. The bigger reason I'd pause is kava's rare link to liver problems. Please run this combo by your pharmacist or doctor before mixing them.

Esomeprazole Strontium also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Good news here: this is a very minor concern. The thinking is that kava might slow down an enzyme called CYP3A4, which helps your body process esomeprazole. In theory, if that enzyme were blocked, a little more of the drug could hang around. But the better-quality human research tells a different story. In a real study, people taking a solid dose of kava for nearly a month showed no meaningful change in CYP3A4 activity.

So in practice, kava is unlikely to make your esomeprazole act stronger or cause extra side effects like headache or stomach upset. My bigger caution is actually kava itself, since it has been tied to rare liver problems. Please run any kava use by your pharmacist or doctor first.
