Why this applies to Estrogen

Estrogen falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist with kava and estrogen. Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs around and out. In a test tube, kava seemed to slow that pump down. If that happened in real life, a bit more estrogen could hang around, which in theory might nudge up estrogen-type effects like breast tenderness, bloating, nausea, or headaches.

The good news is the human evidence is reassuring. When healthy volunteers took kava, it did not change blood levels of a similar pump-handled drug, so this concern is mostly lab-based rather than something we see in people. The bigger reason to be careful is kava's rare link to liver injury. Since the liver helps process estrogen too, it's worth running this combo by your pharmacist or doctor before mixing them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the gist for kava and your estrogen: some of estrogen's breakdown happens through a liver enzyme called CYP1A2. The theory is that kava might slow that enzyme down, which could let estrogen build up a bit and, in theory, nudge up side effects like breast tenderness, bloating, nausea, or headaches.

The good news is that the better-quality human research doesn't back this up. In a study where healthy people took a real kava dose for 28 days, CYP1A2 activity didn't change. So this is a minor, unlikely concern based mostly on lab tests and one case report. The bigger thing to keep in mind is kava's rare risk of liver injury, so please check with your pharmacist or doctor before combining them.

Here's the short version on combining kava with estrogen: this one looks pretty minor.
