Why this applies to Cisapride

Cisapride falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the idea behind this pairing. Your body uses a kind of cellular pump called P-glycoprotein to move drugs like cisapride out of cells. In test-tube studies, kava seemed to slow that pump down, which could let a little more cisapride hang around. The catch is that a human study using kava (225 mg of kavalactones daily) found no real change in a similar pumped drug, so this concern is mostly theoretical for now.

Why does this matter for cisapride specifically? It's a drug that can affect heart rhythm (a QT issue) when levels run high, which is why it was pulled from regular pharmacy shelves. Even a small chance of raising its levels deserves respect. Please loop in your pharmacist or doctor before combining these.

Cisapride also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the practical picture: cisapride (Propulsid) is cleared from your body by a liver enzyme called CYP3A4. The concern on paper is that kava might slow that enzyme down, which could let cisapride build up. That matters because cisapride is sensitive to higher levels, and too much can affect your heart's rhythm (a known reason this drug is rarely used today).

That said, the actual evidence here is reassuring. While early lab tests hinted kava might block CYP3A4, a better human study showed kava taken for 28 days had no real effect on this enzyme. So this interaction is rated minor and unlikely. Still, because cisapride carries real heart risks, please run any kava use by your pharmacist or doctor first.
