Calcifediol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist for Calcifediol (Rayaldee) and kava. Calcifediol is processed in part by a liver enzyme called CYP3A4. The idea is that kava might slow that enzyme down, which could let a bit more calcifediol stick around. In theory, that might nudge your calcium levels higher, leading to things like nausea, constipation, extra thirst, or frequent urination.

But honestly, the evidence here is weak. Test-tube studies hinted at this, yet a real human study showed kava did not meaningfully affect CYP3A4. So this interaction is rated minor and unlikely. The bigger concern with kava is its rare risk of liver damage, especially relevant for kidney patients. Please run any kava use past your pharmacist or doctor first.

What Is Calcifediol Used For?

Brand name: Rayaldee

Rayaldee (calcifediol extended-release capsule) is approved to treat secondary hyperparathyroidism — a condition where the parathyroid glands overproduce parathyroid hormone (PTH) — in adults who have stage 3 or 4 chronic kidney disease and vitamin D levels below 30 ng/mL.

Rayaldee is not approved for stage 5 chronic kidney disease or for people on dialysis (end-stage renal disease).
