Why this applies to Crizotinib

Crizotinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist with kava and your crizotinib (Xalkori). Crizotinib is moved around your body partly by a pump called P-glycoprotein. In test-tube studies, kava seemed to slow that pump down, which in theory could let a bit more crizotinib hang around and add to side effects like nausea, vision changes, fatigue, swelling, or effects on your liver and heart rhythm. But this is mostly lab data. When researchers tested kava in real people using a similar pump-dependent drug, they saw no meaningful change.

So the worry here is mostly theoretical, not proven. Still, since crizotinib is a serious cancer drug and kava itself can stress the liver, I'd run this by your oncologist or pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Crizotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with crizotinib (Xalkori). Your body clears crizotinib using a liver enzyme called CYP3A4. In a test tube, kava looked like it might slow that enzyme down, which in theory could let crizotinib build up and raise the chance of side effects like nausea, vision changes, swelling, or heart rhythm shifts. But when researchers actually tested kava in people for 28 days, it did not meaningfully change CYP3A4 activity. So this concern is mostly theoretical and rated minor and unlikely.

My bigger real-world worry is the liver. Both kava and crizotinib can stress the liver, so stacking them isn't something I'd do casually. Please loop in your oncologist or pharmacist before adding kava.

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) treats adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for ALK or ROS1 gene changes, as confirmed by an FDA-approved test.

Xalkori is also used for pediatric patients (age 1 and older) and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL), and for adult and pediatric patients (age 1 and older) with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
