Quercetin and Ramelteon

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Quercetin interacts with whole categories of medication. Ramelteon falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

1 of 2

Why this applies to Ramelteon

Ramelteon falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: quercetin in higher supplement doses may gently slow down one of the liver enzymes (CYP2C9) that helps clear ramelteon out of your system. Ramelteon is the active drug itself (not a prodrug that needs activating), so if it's cleared more slowly, a bit more can linger in your body. That could mean its sleep-promoting effect feels stronger, with more next-morning grogginess, dizziness, or a foggy, sluggish feeling.

I want to be honest that this is mostly theoretical for ramelteon. The human evidence comes from other CYP2C9 drugs like diclofenac, not ramelteon directly. Food amounts of quercetin almost certainly won't matter, but concentrated supplements might. If you take both, just check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Ramelteon also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your body uses a liver enzyme called CYP3A4 to break down ramelteon. Quercetin can nudge that enzyme, but the research is mixed. In some studies it slows the enzyme down (which could let more ramelteon build up), and in at least one study it seemed to speed the enzyme up (which could mean less). Either way, this is mostly lab and animal data, so it's more of a theoretical concern than a proven problem.

If quercetin did raise your ramelteon levels, you might feel groggier, sleepier, or dizzy the next morning. Food amounts of quercetin (fruits, onions, tea) are very unlikely to matter. Before starting a concentrated supplement, just check with your pharmacist or doctor so they can keep an eye on how you're sleeping.

“Ramelteon (Rozerem) is a prescription sleep aid, and its 228 interacting supplements call for real care, starting with the nine rated major. The frustrating part is that the things sleepless people typically reach for, kava, CBD, a glass of wine before bed, sit at the heart of the problem.”

  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.
