CNS DEPRESSANTS

Chlorpheniramine Maleate, Hydrocodone Bitartrate falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Vituz contains two ingredients that already calm the body: hydrocodone, an opioid cough medicine, and chlorpheniramine, a sedating antihistamine. Both can make you sleepy and slow your breathing on their own. Kava acts in a similar calming way in the brain, so adding it on top stacks those effects together.
When you combine them, the worry is too much sedation: heavy drowsiness, dizziness, slowed reflexes, confusion, and most importantly slowed or shallow breathing, which can be dangerous with an opioid. Good-quality evidence backs this concern, and expert guidelines say not to mix kava with sedating drugs. There's also kava's own risk of liver injury to keep in mind. Please don't start kava with Vituz without first checking with your pharmacist or doctor.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the thing with Vituz (chlorpheniramine plus hydrocodone) and kava. On paper, hydrocodone is partly turned into its stronger pain-relieving form by a liver enzyme called CYP2D6, and lab dish studies hinted that kava might block that enzyme. If that actually happened in people, you might expect slightly less pain relief from the hydrocodone. But the better human research shows kava doesn't really change how CYP2D6 works in real life, so this particular interaction is rated minor and unlikely.
What I'd pay more attention to is the overlap in sedation. Hydrocodone, chlorpheniramine, and kava can all make you drowsy and slow your breathing. Together that effect can stack up. Please check with your pharmacist or doctor before pairing them, and don't drive until you know how you feel.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Vituz combines hydrocodone (an opioid cough suppressant) with chlorpheniramine (an antihistamine). The lab-based worry here is that kava might block CYP3A4, an enzyme that helps clear hydrocodone. If that were true, hydrocodone could build up and cause more drowsiness, dizziness, or slowed breathing. But the better human research is reassuring: in a real study, kava did not actually change CYP3A4 activity, so this enzyme effect looks unlikely.
The bigger, more practical concern is overlap. Both kava and Vituz are calming and sedating, so combining them can stack up the sleepiness and slow your breathing. Kava can also stress the liver. Please check with your pharmacist or doctor before pairing them.
