Risperidone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the practical scoop on pairing kava with risperidone. Your body uses a liver enzyme called CYP2D6 to help break risperidone down. In lab dishes, kava seemed to slow that enzyme, which in theory could let risperidone build up a bit and bring on more of its usual effects, like drowsiness, dizziness, low blood pressure when standing, or stiffness. But the key word is theory. When this was actually tested in people, kava did not meaningfully change how the body handles CYP2D6 drugs, so a real problem here is unlikely.

My bigger concern with this combo isn't the enzyme at all. Both kava and risperidone can make you sleepy, so stacking them may leave you more drowsy or foggy than expected. And since kava has been tied to rare liver injury, it's worth a quick chat with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist for risperidone and kava. Risperidone is partly processed by a liver enzyme called CYP3A4, and early lab-dish studies hinted that kava might slow that enzyme down. If that were true, more risperidone could linger in your body, which might mean extra drowsiness, dizziness, or low blood pressure. But here's the reassuring part: a more reliable human study found that kava did not actually change CYP3A4 activity. So this concern stays mostly theoretical, and our rating for it is minor and unlikely.

That said, both kava and risperidone can be sedating on their own, so combining them could leave you feeling extra sleepy or foggy. Please check with your pharmacist or doctor before pairing them, especially since kava can rarely stress the liver.

What Is Risperidone Used For?

Brand names: Risperdal, Perseris

Risperidone is prescribed for three main conditions: schizophrenia (a disorder affecting how a person thinks, feels, and perceives reality), acute manic or mixed episodes in Bipolar I Disorder, and irritability associated with autistic disorder — including aggression, self-injury, temper tantrums, and rapid mood swings.

For bipolar mania, it can be used alone or alongside lithium or valproate. It is approved for use in both adults and certain age groups of children and adolescents, depending on the condition being treated.
