Combining kava and haloperidol might increase the risk of cardiovascular adverse effects and hypoxia.
Atrial flutter and hypoxia has been reported for a patient who received intramuscular injections of haloperidol and lorazepam after using kava orally. The side effects were attributed to kava-induced inhibition of CYP2D6, but might also have been related to additive adverse effects with the concomitant use of haloperidol, lorazepam, and kava.

Here's the gist if you're using kava alongside haloperidol (Haldol). Haloperidol is an active medication, not a prodrug, and your body partly clears it using a liver enzyme called CYP2D6. Kava may slow that enzyme down, so the worry is that haloperidol could build up a bit more than expected, nudging up the chance of side effects like drowsiness, stiffness, dizziness, or, in a rare reported case, heart rhythm changes and low oxygen.

I want to be straight with you: this is based on a single anecdotal report (one patient who also got lorazepam), so the evidence is weak. There's also a simple additive concern, since both kava and haloperidol can have calming, sedating effects. On top of that, kava can rarely harm the liver. I'd hold off on combining them until you check with your pharmacist or doctor.

Haloperidol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture: your body uses an enzyme called CYP1A2 to help clear haloperidol, and there was some early worry that kava might slow that enzyme down. If that were true, haloperidol could build up a bit and you might notice more of its usual effects, like drowsiness, stiffness, restlessness, or slowed movements. But the better-quality human study we have showed kava had no real effect on CYP1A2, so this concern looks unlikely to play out in practice.

What I'd watch more closely is overlap in how they feel. Both kava and haloperidol can make you sleepy, so together they may leave you extra groggy. Kava can also stress the liver in rare cases. Please loop in your pharmacist or doctor before combining them so they can weigh it for you.

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the practical picture with kava and haloperidol (Haldol). Your body partly clears haloperidol using a liver enzyme called CYP2D6. In a test tube, kava looked like it could slow that enzyme down, which in theory might let haloperidol build up and bring stronger effects like deep drowsiness, dizziness, muscle stiffness, or restlessness. But here's the reassuring part: actual studies in people showed kava did not meaningfully change how this enzyme works, so this concern stayed in the lab.

My bigger real-world worry is overlap, not metabolism. Both kava and haloperidol can be sedating, and kava has been tied to rare liver injury. If you take both, mention it to your pharmacist or doctor before combining them.

Here's the good news on this one. Haloperidol (Haldol) is partly broken down by a liver enzyme called CYP3A4. On paper, kava was once thought to slow that enzyme down, which could let haloperidol build up. But the better-quality human studies don't back that up. In one study, healthy people took kava for 28 days and it didn't actually change CYP3A4 activity. So this particular concern is rated as minor and unlikely.

That said, I'd still raise a bigger flag for a different reason. Both kava and haloperidol can be sedating, so taken together you may feel extra drowsy, foggy, or dizzy. Kava has also been tied to rare liver problems. Please loop in your pharmacist or doctor before combining them, and don't change your haloperidol on your own.
