Ashwagandha + Ramelteon
Ramelteon brand name: Rozerem.
Ramelteon falls into 3 drug categories that Ashwagandha can interact with; the most serious is rated moderate. Theoretically, taking ashwagandha might increase the sedative effects of CNS depressants.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Ashwagandha interacts with whole categories of medication. Ramelteon falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CNS DEPRESSANTS
1 of 3Why this applies to Ramelteon
Ramelteon falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind if you're pairing ashwagandha with Ramelteon (Rozerem). Ramelteon is a sleep aid that works on melatonin receptors to help you drift off, and ashwagandha is thought to have its own calming, sedative quality. Put them together and you could feel sleepier than expected, with effects like next-morning grogginess, dizziness, or feeling a bit foggy.
I want to be honest about the evidence here: this is a theoretical concern based on how each one behaves, not something proven in studies. For many people it may not cause any real problem, but since both are nudging you toward sleep, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you notice extra drowsiness.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 3Ramelteon also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two. Your body relies on a liver enzyme called CYP1A2 to break down ramelteon. In lab studies (not in people yet), ashwagandha seemed to speed that enzyme up. If that holds true in the body, it could clear ramelteon a little faster and leave less of it working, which might make it a touch less effective at helping you drift off to sleep.
I want to keep this in perspective, though. This is rated a minor interaction based only on test-tube research, so it may not matter much in real life. If you notice ramelteon isn't helping you fall asleep like it used to after adding ashwagandha, mention it to your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Ramelteon also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with ramelteon (Rozerem) and ashwagandha. Ramelteon is a melatonin-type sleep aid that your liver clears using an enzyme called CYP3A4. In lab studies, ashwagandha appears to speed up that enzyme, which could in theory clear ramelteon out a little faster and make it slightly less effective at helping you drift off.
So if anything, the concern is a weaker sleep effect, not a stronger one. The good news: this is rated as a minor interaction based only on test-tube research, so it may not amount to much in real life. Still, since both are taken for sleep, it's worth a quick chat with your pharmacist or doctor before combining them so you're not working against yourself.

From our pharmacist’s overview on the Ramelteon page
“Ramelteon (Rozerem) is a prescription sleep aid, and its 228 interacting supplements call for real care, starting with the nine rated major. The frustrating part is that the things sleepless people typically reach for, kava, CBD, a glass of wine before bed, sit at the heart of the problem.”
