Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Tuzistra XR pairs codeine (a narcotic cough suppressant) with chlorpheniramine (a sedating antihistamine). Both already slow down your brain and can make you sleepy. Kava adds its own calming, sedative punch on top of that, so stacking them together can leave you noticeably more drowsy, foggy, dizzy, and slow to react.

The bigger worry is that the deepest layer of these medicines, codeine, can slow your breathing. Kava may quietly push that further, especially at higher doses or if you also drink alcohol. This is a well-supported interaction, and expert guidelines say to avoid mixing kava with sedating medicines. Please don't change anything on your own. Just check with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Tuzistra XR pairs codeine with the antihistamine chlorpheniramine. Here's a wrinkle: codeine is a prodrug, meaning your body has to convert it (using the liver enzyme CYP2D6) into morphine before it actually relieves your cough or pain. In a test tube, kava looked like it could block CYP2D6, which in theory would leave you with less pain relief from the codeine. But the actual human studies tell a reassuring story: kava didn't meaningfully change how people process CYP2D6 medications. So this particular interaction is rated minor and unlikely.

That said, both kava and this combo can make you drowsy and foggy, so stacking them could leave you more sedated than you'd expect. Kava can also stress the liver. I'd run it by your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Tuzistra XR combines codeine with chlorpheniramine, and the worry on file is whether kava changes how your body handles the codeine portion through a liver enzyme called CYP3A4. The good news here is that the better human research found kava had no real effect on this enzyme. The lab studies that suggested a problem haven't held up in actual people, so this interaction is rated minor and unlikely.

That said, both kava and this cough syrup can leave you feeling drowsy, foggy, or extra sleepy, and stacking two calming things can deepen that. There's also kava's known risk of liver harm to keep in mind. Please check with your pharmacist or doctor before combining them, especially if you'll be driving.
