Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and Adagrasib (Krazati). This isn't about kava changing how your cancer medicine is broken down. Instead, it's about both being hard on the same organ: your liver. Adagrasib is already known to raise liver enzymes and can sometimes cause real liver injury, which is why your oncology team checks bloodwork along the way. Kava has its own track record of rare but serious liver damage, mostly with heavy or long-term use.

Stacking two things that stress the liver could add up. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. To be safe, please loop in your oncologist or pharmacist before using kava. Don't change your Adagrasib on your own.

Adagrasib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

If you're taking Adagrasib (Krazati) for your lung cancer, here's the practical picture with kava. Adagrasib is handled by a liver enzyme called CYP3A4. The worry on paper is that kava might block that enzyme, which could let Adagrasib build up and raise the chance of side effects like nausea, diarrhea, tiredness, or heart rhythm changes (QT prolongation).

The good news: this concern comes mostly from test-tube studies. A real human study found that kava had no meaningful effect on CYP3A4. So an actual interaction is considered minor and unlikely. Still, Adagrasib is a serious cancer drug, and kava itself can stress the liver. Please check with your oncologist or pharmacist before combining them.

About Adagrasib

What Is Adagrasib Used For?

Brand name: Krazati

Krazati (adagrasib) is used to treat adults with locally advanced or metastatic non-small cell lung cancer (NSCLC) that carries a specific genetic change called a KRAS G12C mutation. It is used as a single agent for NSCLC after at least one prior treatment, and patients must be confirmed to have this mutation by an FDA-approved test.

Krazati is also used in combination with another drug called cetuximab to treat adults with KRAS G12C-mutated locally advanced or metastatic colorectal cancer (CRC) who have already been treated with specific prior chemotherapy regimens. Both uses were approved under the FDA's accelerated approval program based on tumor response data, and continued approval may depend on results from confirmatory trials.
