Belinostat falls under HEPATOTOXIC DRUGS — the group Kava interacts with.

Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and belinostat (Beleodaq). This isn't about kava changing how belinostat works against your lymphoma. It's about your liver. Belinostat can already be hard on the liver, and your care team watches your liver enzymes closely while you're on it. Kava has its own track record of rare but real liver injury, mostly with heavy or long-term use.

Stack two things that can stress the liver and you may raise the odds of trouble, like fatigue, nausea, yellowing of the skin or eyes, or dark urine. To be fair, this is mostly theoretical and based on scattered case reports, not strong studies. Still, with a serious chemo drug, please clear any kava use with your oncologist or pharmacist first.

Belinostat also falls under P-GLYCOPROTEIN SUBSTRATES.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the simple version. Belinostat (Beleodaq) is your chemo infusion for T-cell lymphoma, and it's moved around the body in part by a transporter called P-glycoprotein, or P-gp. The idea on file is that kava might block that transporter. If it did, a bit more belinostat could hang around, which in theory could mean more of its known side effects, like nausea, tiredness, low blood counts, or infection risk.

That said, the evidence here is weak. It comes mostly from lab dish studies, and a human study found kava didn't change levels of a similar P-gp drug. So this is more theoretical than proven. The bigger real concern is that both kava and belinostat can stress the liver. Please talk with your oncology team before using kava.

About Belinostat Injection

What Is Belinostat Injection Used For?

Brand name: Beleodaq

Beleodaq (belinostat) injection is approved to treat adult patients with relapsed or refractory peripheral T-cell lymphoma (PTCL) — a fast-growing type of lymphoma (blood cancer) that either came back after treatment or never fully responded to it.

This approval is based on how well the cancer responded to the drug, including how long those responses lasted. Whether Beleodaq extends overall survival or improves disease-related symptoms has not yet been established. Continued approval may depend on results from a confirmatory trial.
