Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing about Zileuton: it's already known to be tough on the liver. That's why your doctor checks your liver enzymes with blood tests when you start it and periodically afterward. Kava has its own track record of rare but serious liver injury, mostly with heavy or long-term use. Put two liver-stressing things together and you may stack the odds against your liver.

I want to be honest that this concern is mostly theoretical and based on scattered case reports, not strong studies. Still, it's worth respecting. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness, and call your doctor right away if they show up. Please check with your pharmacist or doctor before combining these two.

Theoretically, kava might increase levels of CYP2C9 substrates.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Here's the gist for Zileuton (Zyflo). Your liver uses an enzyme called CYP2C9 to help clear this medicine. In lab dish studies, kava seems to slow that enzyme down. If that happened in real life, zileuton could build up a bit and lead to stronger side effects, like nausea, stomach upset, headache, or changes in your liver tests.

I want to be honest about the evidence though: this is only seen in test-tube research, not in actual people, so it's mostly theoretical. The bigger concern is that both kava and zileuton can stress the liver. Stacking two things that affect the liver isn't ideal. Please check with your pharmacist or doctor before pairing them, and don't change your asthma medicine on your own.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version: this combo is mostly a theoretical concern, not something the better research backs up. Your body uses an enzyme called CYP1A2 to help clear Zileuton. Some lab studies and one case report hinted that kava might slow that enzyme down, which in theory could let Zileuton build up a little and possibly raise the chance of side effects like nausea, stomach upset, or headache. But a well-run human study found kava had no real effect on CYP1A2, so the worry mostly doesn't hold up.

One thing that does matter more: both kava and Zileuton can stress the liver. Since Zileuton already requires liver monitoring, it's worth running this past your pharmacist or doctor before combining them.

Here's the gist for Zileuton (Zyflo) and kava. Zileuton is partly handled by a liver enzyme called CYP3A4. The thought was that kava might slow that enzyme down, which in theory could let zileuton build up and make side effects (like nausea, stomach upset, or headache) a bit more likely. But the better-quality human research doesn't back that up. In one study, healthy people took kava daily for 28 days and it didn't change CYP3A4 activity at all.

So this is rated minor and unlikely. The bigger thing I'd flag is your liver. Both kava and zileuton can stress the liver, so combining them isn't something I'd do without checking first. Please talk with your pharmacist or doctor before pairing these.

About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
