Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing to know about pairing kava with Avacopan (Tavneos): this isn't about one changing how the other works in your blood. It's about your liver. Avacopan can put some stress on the liver on its own, which is why your doctor checks your liver enzymes with blood tests during treatment. Kava has its own track record of rare but real liver injury, mostly with heavy or long-term use.

Stack the two together and you may add to that strain. Watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. To be fair, this concern is theoretical and based on scattered case reports, not strong studies. Still, please check with your doctor or pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Avacopan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version for Avacopan (Tavneos): your body relies on a liver enzyme called CYP3A4 to clear this medication. There was an early lab worry that kava might slow that enzyme down, which in theory could let Avacopan build up and lead to more side effects like nausea, headache, or higher liver enzymes. But when researchers actually tested kava in real people, it did not meaningfully change CYP3A4 activity. So this interaction is considered minor and unlikely.

My bigger concern is the overlap on your liver. Both kava and Avacopan can stress the liver, so combining them isn't something I'd do casually. Please check with your pharmacist or doctor before adding kava, and don't change your Avacopan on your own.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
