CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
Why this applies to Bendamustine Hydrochloride
Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version on combining kava with bendamustine (Bendeka). Bendamustine is partly processed by a liver enzyme called CYP1A2, and there was a theory that kava might block that enzyme. If that were true, bendamustine could build up and cause more of its usual side effects, like low blood counts, nausea, fatigue, and a higher infection risk. The good news is that the better-quality human research found kava had no real effect on CYP1A2, so this concern is mostly theoretical and unlikely to matter.
That said, my bigger worry is that both kava and chemotherapy can stress the liver. Since you're being treated for a serious cancer, please loop in your oncology team or pharmacist before adding kava. Don't change anything on your own.

What Is Bendamustine Injection Used For?
Brand name: Bendeka
Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.
For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
