Why this applies to Bendamustine Hydrochloride

Bendamustine Hydrochloride falls under ALKYLATING AGENTS — the group Turmeric interacts with.

Turmeric has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals. However, research is conflicting.
In vitro research suggests that curcumin, a constituent of turmeric, inhibits mechlorethamine-induced apoptosis of breast cancer cells by up to 70%. Also, animal research shows that curcumin inhibits cyclophosphamide-induced tumor regression. However, some in vitro research shows that curcumin does not affect the apoptosis capacity of etoposide. Also, other laboratory research suggests that curcumin might augment the cytotoxic effects of alkylating agents. Reasons for the discrepancies may relate to the dose of curcumin and the specific chemotherapeutic agent. Lower doses of curcumin might have antioxidant effects while higher doses might have pro-oxidant effects. More evidence is needed to determine what effect, if any, turmeric might have on alkylating agents.

Here is the idea behind this caution. Bendamustine is a chemo drug that helps kill cancer cells partly by creating cell-damaging molecules called free radicals. Curcumin, the active part of turmeric, can act as an antioxidant, which means in theory it could mop up some of those free radicals and blunt how well the chemo works against your leukemia or lymphoma.

I want to be honest with you though: this concern comes from lab and animal studies, not from real patients, and the results actually point in different directions. Some studies even suggest curcumin might boost chemo's effect. So we really do not know what happens in people. Cooking with turmeric in food is very unlikely to matter, but concentrated supplements are a different story. Please do not start or stop turmeric without first checking with your oncology team.

Theoretically, turmeric might increase levels of drugs metabolized by CYP1A2. However, research is conflicting.
In vitro and animal research show that the turmeric constituent, curcumin, inhibits CYP1A2. However, other in vitro research suggests that curcumin does not significantly affect CYP1A2.

Here's the situation with turmeric and your bendamustine (Bendeka). Bendamustine is partly handled by a liver enzyme called CYP1A2. Lab and animal studies suggest the curcumin in turmeric might slow that enzyme down a bit, which in theory could let more bendamustine linger in your body. If that happened, you could in principle see stronger side effects like low blood counts, nausea, fatigue, or a higher infection risk.

The honest truth, though, is that this is theoretical and weak evidence. The research even disagrees with itself, and small amounts of turmeric used in cooking are very unlikely to cause a problem. Concentrated curcumin supplements are a different story. Since bendamustine is serious chemotherapy, please run any turmeric supplement past your oncologist or pharmacist first.

What Is Bendamustine Injection Used For?

Brand name: Bendeka

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
