Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Kava interacts with.
In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days inhibited the metabolism of CYP2E1 substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the honest take on combining kava with this cough and cold combo. The interaction on file is about kava slowing down an enzyme called CYP2E1. The thing is, neither chlorpheniramine (the antihistamine) nor dextromethorphan (the cough suppressant) relies much on CYP2E1, so this particular pathway probably isn't where the real concern lies. The bigger, more practical issue is overlap in effects.
Kava is calming and so is chlorpheniramine, so stacking them can leave you extra drowsy, foggy, or dizzy. Dextromethorphan can add to that sedation too. I'd also flag kava's link to rare liver injury as a reason to be careful. Nothing here means you can't be helped, but please run this pairing by your pharmacist or doctor first.

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the practical picture with this cough and cold combo (chlorpheniramine plus dextromethorphan) and kava. Dextromethorphan is processed by a liver enzyme called CYP2D6. In a test tube, kava looked like it could slow that enzyme down, which in theory could let dextromethorphan build up. But the actual human studies didn't show kava meaningfully changing how the body handles CYP2D6 drugs, so this is rated minor and unlikely.
My bigger real-world concern is overlap, not metabolism. Both kava and this product can make you drowsy, foggy, or dizzy, and chlorpheniramine already causes sedation. Stacking them may leave you more wiped out than expected. Kava can also stress the liver, so it's worth a quick chat with your pharmacist or doctor before combining them.

Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. Here's the gist with this cough-and-cold combo (which pairs the antihistamine chlorpheniramine with the cough suppressant dextromethorphan) and kava. On paper, kava was flagged because it might slow the CYP3A4 enzyme that helps clear these ingredients.
