CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Aspirin, Chlorpheniramine, Phenylpropanolamine
Aspirin, Chlorpheniramine, Phenylpropanolamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Good news here: the enzyme angle (CYP3A4) for this combo product is mostly a non-issue. Early lab tests hinted that kava might slow that enzyme, which could in theory let drugs build up, but a better human study found kava had no real effect on CYP3A4. So I wouldn't expect kava to meaningfully change how your body handles this cold medicine.
What I'd watch instead is overlap in effects. The chlorpheniramine in Alka-Seltzer Plus Cold causes drowsiness, and kava is calming too, so together they may leave you more sleepy or foggy than usual. There's also a separate concern worth knowing: both kava and aspirin can be hard on the liver and stomach, respectively. Run this pairing by your pharmacist before mixing them.
