P-GLYCOPROTEIN SUBSTRATES
Why this applies to Alvimopan
Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining kava with alvimopan (Entereg). Alvimopan is carried around by a transporter called P-glycoprotein, which acts like a tiny pump that moves the drug out of cells. The concern on paper is that kava might slow that pump down, which could let a bit more alvimopan stay in your system than expected.
The good news is that the evidence here is weak. The lab (test tube) studies hint at this effect, but a human study using a similar pump-dependent drug showed no real change. So this is mostly theoretical. Still, alvimopan is usually a short course given right around bowel surgery in a hospital, and kava carries its own liver risks. Please check with your pharmacist or surgical team before mixing the two.

What Is Alvimopan Used For?
Brand name: Entereg
Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.
This medication is only given during a short hospital stay and is not for long-term or home use.
