Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing to keep an eye on with kava and ambrisentan (Letairis, Volibris): both can be tough on your liver. Ambrisentan already requires regular liver checks because it can occasionally raise liver enzymes, and kava has been tied to rare but serious liver injury. Stacking two liver stressors together may add up, which is the concern here.

I'll be honest that this is mostly a theoretical worry based on scattered case reports, not strong studies, and most kava liver problems happened with heavy, long-term use. Still, with a medication like this I'd play it safe. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please talk with your doctor or pharmacist before combining them.

Ambrisentan also falls under P-GLYCOPROTEIN SUBSTRATES.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist for ambrisentan (Letairis). Kava might block a transporter called P-glycoprotein, which is one of the doormen that helps shuttle ambrisentan out of your body. If kava slows that doorman down, a bit more ambrisentan could hang around, which in theory might nudge up side effects like swelling in your legs, a stuffy nose, flushing, or feeling lightheaded.

The good news is that this is mostly lab-dish thinking. When researchers gave people kava with digoxin (another drug handled by this same transporter), nothing meaningful happened. So a real-world problem is far from certain, though higher kava doses are less studied. One bigger caution: kava has been tied to rare liver injury, and ambrisentan can also stress the liver, so please check with your pharmacist or doctor before combining them.

Ambrisentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on combining kava with your ambrisentan (Letairis). Ambrisentan is partly handled by a liver enzyme called CYP3A4. Early test-tube studies hinted that kava might slow this enzyme, which in theory could let ambrisentan build up a bit and raise the chance of side effects like swelling in your legs, a stuffy nose, headache, or feeling flushed. But a better human study found kava actually had no real effect on CYP3A4, so this interaction looks unlikely to cause problems in practice.

My bigger concern is your liver, not the enzyme. Kava has been linked to rare but serious liver injury, and ambrisentan also requires liver monitoring. Please check with your pharmacist or doctor before adding kava.

About Ambrisentan

What Is Ambrisentan Used For?

Brand names: Letairis, Volibris

Ambrisentan tablets and Letairis tablets are both used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — a condition where the blood pressure in the arteries leading to the lungs is dangerously high. Both are approved to help adults exercise better and slow the worsening of PAH.

Both ambrisentan tablets and Letairis are also approved to be used together with tadalafil (another PAH medicine) to further reduce the risk of disease progression, lower the chance of hospitalization for worsening PAH, and improve exercise ability. Most of the clinical evidence was in patients with mild-to-moderate symptoms and PAH caused by unknown or inherited causes, or PAH linked to connective tissue diseases.
