Thioridazine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Lab tests suggest kava may slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to clear thioridazine. If that enzyme runs slower, thioridazine could build up a bit higher than usual, which may mean stronger effects and more side effects like drowsiness, dizziness, dry mouth, or a faster heartbeat.
What's important: thioridazine carries a known warning about heart rhythm changes (QT prolongation), so higher levels are something to take seriously. That said, this is only seen in test-tube research so far, not in real people, so it's theoretical. Please don't change anything on your own. Talk with your pharmacist or doctor before combining these, especially since kava can also stress the liver.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for thioridazine (Mellaril): your liver uses an enzyme called CYP2D6 to clear it out. In a test tube, kava looks like it might block that enzyme, which in theory could let thioridazine build up and cause stronger side effects like drowsiness, dizziness, dry mouth, low blood pressure, or heart rhythm changes. But here's the reassuring part: when researchers actually studied this in real people, kava did not meaningfully change how the body handles CYP2D6 drugs. So this concern stays mostly theoretical.
The bigger thing I'd flag is that both kava and thioridazine can be hard on the body in their own ways. Please run this combination by your pharmacist or doctor before adding kava, rather than starting or stopping anything on your own.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. What Is Thioridazine Used For?
Brand name: Mellaril
Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.
Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
