Why this applies to Rucaparib Camsylate

Rucaparib Camsylate falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist: your body uses a kind of cellular bouncer called P-glycoprotein (P-gp) to push certain drugs back out, and Rucaparib (Rubraca) is one of those drugs that gets handled by it. In a test tube, kava seemed to slow that bouncer down. If that happened in real life, a bit more rucaparib could hang around, which in theory might mean more of its usual side effects like nausea, fatigue, low blood counts, or changes in liver-related lab values.

The reassuring part: this concern comes from lab studies, and a human study with digoxin (another P-gp drug) showed no real effect from kava. So this is mostly theoretical. Still, kava itself can stress the liver, which matters with cancer therapy. Please check with your oncology team or pharmacist before combining them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with your Rubraca (rucaparib): the concern on file is that kava might slow down a liver enzyme called CYP2D6, which is one of the pathways involved in clearing this medication. In a test tube, kava did seem to block that enzyme. But in actual human studies, kava did not meaningfully change how the body handles CYP2D6 drugs. So in practice, this is rated minor and unlikely to cause a real problem.

That said, I'd still raise it with your oncology team. My bigger worry isn't the enzyme, it's that both kava and rucaparib can stress the liver. Rucaparib already needs regular liver and blood checks, so adding kava (which carries a rare risk of serious liver injury) isn't something I'd do without your doctor's okay.

About Rucaparib

What Is Rucaparib Used For?

Brand name: Rubraca

Rubraca (rucaparib) is used for two types of cancer in adults who carry a harmful BRCA gene mutation (either inherited or found in the tumor). First, it is used as maintenance treatment — meaning ongoing treatment to keep cancer from coming back — in people with recurrent epithelial ovarian cancer, fallopian tube cancer, or primary peritoneal cancer who responded (fully or partially) to platinum-based chemotherapy.

Second, Rubraca treats metastatic castration-resistant prostate cancer (advanced prostate cancer that has stopped responding to hormone-lowering treatment) in adults who have already received androgen receptor-directed therapy. Patients must be tested with an FDA-approved companion diagnostic to confirm the BRCA mutation before starting Rubraca for prostate cancer.
