Here's the situation with kava and your leukemia medicine, Idhifa (enasidenib). Lab studies show kava can block a liver enzyme called CYP2C19, which helps process certain drugs. In theory, slowing that enzyme could let enasidenib build up a bit higher in your body, possibly nudging up side effects like nausea, diarrhea, low appetite, or changes in your blood counts.

I want to be honest though: this is only seen in test-tube research, not in real patients, so we don't know if it actually happens in people. There's a bigger reason to be cautious here, both kava and enasidenib can stress the liver, so combining them isn't ideal. Please check with your oncology team or pharmacist before adding kava.

Here's the short version on combining kava with enasidenib (Idhifa). Your body uses a liver enzyme called CYP2C9 to help process enasidenib. In test-tube studies, kava appears to slow that enzyme down. If that happened in people, enasidenib could build up a bit more than expected, which in theory might mean stronger side effects like nausea, diarrhea, or worsening lab values such as bilirubin.

The important caveat: this is based only on lab research, not on real patients, so it's still theoretical. I'd also point out that kava itself can stress the liver, and enasidenib already requires liver monitoring, so that overlap is worth a conversation. Please don't start or stop anything on your own. Check with your oncology team or pharmacist first.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version: this is a pretty minor concern. Enasidenib (Idhifa) is handled in part by a liver enzyme called CYP1A2, and the worry on paper is that kava might slow that enzyme down, which could let a bit more of the drug build up. But the better-quality human research tells a calmer story. In one study, healthy people took a real kava dose for 28 days and saw no change in CYP1A2 activity, so any effect on enasidenib is unlikely.

The bigger thing I'd flag isn't this enzyme at all. Both kava and enasidenib can stress the liver, so stacking them may add up. If you're on enasidenib for your leukemia, please check with your oncology team or pharmacist before adding kava.

Here's the short version: the worry on paper is that kava might slow down a liver enzyme called CYP2D6, which in theory could let drugs handled by that enzyme build up a bit. But for enasidenib (Idhifa), this is really a minor, unlikely concern. While test-tube studies hinted that kava blocks CYP2D6, actual human studies showed kava did not meaningfully change how the body clears CYP2D6 drugs. So I wouldn't expect kava to make enasidenib stronger or its side effects (like nausea, low appetite, or rising bilirubin) noticeably worse through this pathway.

My bigger caution is liver health. Both kava and enasidenib can stress the liver, so please loop in your oncologist or pharmacist before adding kava.

Here's the practical bottom line for your leukemia medicine. Enasidenib (Idhifa) is processed in part by a liver enzyme called CYP3A4, and kava was once thought to slow that enzyme down based on test-tube studies. The worry was that kava might let enasidenib build up. But in real people, a careful clinical study found that kava did not actually change CYP3A4 activity. So this concern looks mostly theoretical, and a real interaction is considered unlikely and minor.

The bigger thing I'd flag is that kava itself has been tied to rare liver injury, and enasidenib already carries its own risks (including effects on liver and blood counts). Because of that overlap, please loop in your oncology team or pharmacist before adding kava.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
