Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with combining kava and sotorasib (Lumakras): both can be tough on your liver. Sotorasib already carries a known risk of liver injury, and your oncology team likely checks your liver blood tests (like ALT and AST) regularly because of that. Kava has been tied to over 100 reports of liver damage, mostly with heavy or long-term use. Stacking these two together could, in theory, raise the odds of liver trouble.

To be fair, this is based on anecdotal reports, not strong studies, so we're being cautious rather than alarmed. Still, when you're on a cancer medicine that's hard on the liver, adding kava isn't worth the gamble. Please don't change anything on your own. Talk with your oncologist or pharmacist first.

Sotorasib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with sotorasib (Lumakras). Sotorasib is partly broken down by a liver enzyme called CYP3A4. Some early lab tests hinted that kava might slow that enzyme down, which in theory could let sotorasib build up and possibly increase side effects like nausea, diarrhea, fatigue, or liver enzyme changes. But here's the reassuring part: a better-quality study in real people found that kava did not actually affect CYP3A4. So this concern is mostly theoretical and unlikely to cause a real problem.

My bigger flag is the liver. Both kava and sotorasib can stress the liver, and your cancer team already watches your liver labs closely. Please don't add kava without checking with your oncologist or pharmacist first.

About Sotorasib

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved under the FDA's accelerated approval pathway based on tumor response rates, and continued approval may depend on results from confirmatory trials.

Lumakras is also used in combination with panitumumab to treat adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have previously received certain chemotherapy regimens. In both cases, patients must first be confirmed to carry the KRAS G12C mutation using an FDA-approved test.
