THC, the part of cannabis that gets you high, is partly broken down by a liver enzyme called CYP2C9. Enasidenib can slow that enzyme down a bit, so the THC may stick around longer and hit harder than usual.

In vitro research shows that the cannabis constituents delta-9-tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol moderately inhibit the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the idea behind this pairing. Your body partly clears Enasidenib using a liver enzyme called CYP2C9. Compounds in cannabis (THC, CBD, and others) can mildly slow that same enzyme down. In theory, if that enzyme works slower, more Enasidenib could linger in your system, which might nudge up its effects and side effects, things like nausea, diarrhea, low appetite, or changes in your blood counts and liver tests.

I want to be honest about the strength of this, though. This concern comes only from lab (test-tube) studies, not from real patients, so it's more theoretical than proven. Here's the simple version: your body uses an enzyme called CYP3A4 to help break down and clear enasidenib (Idhifa). Lab studies suggest that compounds in cannabis (especially CBD and CBG) can slow that enzyme down a bit. If enasidenib is cleared more slowly, its levels could creep up, which in theory might mean more of its known side effects, things like nausea, diarrhea, low appetite, fatigue, or elevated bilirubin. Since enasidenib also carries a risk of a serious reaction called differentiation syndrome, you don't want extra surprises.

Now the honest part: this is only a theoretical concern based on test-tube research, not real patient studies, so it may not amount to much. Still, with a cancer drug like this, please loop in your oncologist or pharmacist before mixing in cannabis.

Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
