Fedratinib Hydrochloride falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabis interacts with.

Cannabis may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the gist for fedratinib (Inrebic): your body partly relies on a liver enzyme called CYP2C19 to help clear this medication. The CBD in cannabis can slow that enzyme down. In theory, that could let fedratinib build up a bit higher in your blood than expected, which may mean more of its side effects rather than a weaker effect. Fedratinib is the active drug itself, not a prodrug, so more in the system generally means stronger effects.

With fedratinib that could look like worse nausea, diarrhea, vomiting, fatigue, or low blood counts. To be clear, this concern is theoretical and based on how these compounds behave, not on direct studies with fedratinib. Still, since this is a cancer medicine with careful monitoring, please run any cannabis or CBD use by your oncologist or pharmacist first.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist for fedratinib (Inrebic). Your body clears this medication largely through a liver enzyme called CYP3A4. Some lab studies suggest cannabis (especially CBD and CBG) can slow that enzyme down a bit. If that happens in real life, fedratinib could clear more slowly and build up, which may make its side effects more noticeable. Think along the lines of nausea, diarrhea, fatigue, low blood counts, or worsening of the dangerous brain side effect (encephalopathy) this drug carries a warning for.

I want to be honest though: this is theoretical, based only on test-tube research, not real patient cases. Still, fedratinib is a serious cancer drug, so please loop in your oncologist or pharmacist before adding cannabis.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
