Acetaminophen, Butalbital, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.

Here's the honest picture with this combo pill (which mixes acetaminophen, the barbiturate butalbital, and the opioid codeine). The concern on file is that Rhodiola might block a liver enzyme called CYP1A2 in lab dishes. In theory, slowing that enzyme could let certain drugs build up a bit. But this is mostly test-tube data, and a real human study showed Rhodiola didn't actually slow caffeine, another CYP1A2 drug. So the practical risk here looks small.

On top of that, CYP1A2 isn't the main pathway for any of these three ingredients, so I wouldn't expect a meaningful change in how this medicine works. The bigger thing I'd watch is overlap in sleepiness, since both Rhodiola and this pill can affect how you feel. If you notice extra drowsiness, dizziness, or foggy thinking, check in with your pharmacist or doctor before combining them regularly.

A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.

This combo pill (Phrenilin with codeine) carries two ingredients that calm the brain: butalbital, a barbiturate, and codeine, an opioid. Both can make you sleepy, slow your breathing, and lower your blood pressure. The concern here is simply additive: rhodiola may add a little to that calming effect, so the worry is feeling more drowsy, foggy, dizzy, or unsteady than usual.

I'll be honest that this is a theoretical caution built on a handful of case reports, mostly with stronger sedatives like diazepam and alprazolam, not with this exact medicine. It's rated minor. Still, since these tablets already pack a sedating punch, it's worth a quick chat with your pharmacist or doctor before adding rhodiola, especially before driving.
