Bendamustine falls under ALKYLATING AGENTS — the group Vitamin C interacts with.

Theoretically, antioxidant effects of vitamin C might reduce the effectiveness of alkylating agents.
The use of antioxidants like vitamin C during chemotherapy is controversial. There is concern that antioxidants could reduce the activity of chemotherapy drugs that generate free radicals, such as cyclophosphamide, chlorambucil, carmustine, busulfan, and thiotepa. In contrast, some researchers theorize that antioxidants might make chemotherapy more effective by reducing oxidative stress that could interfere with apoptosis (cell death) of cancer cells. More evidence is needed to determine what effect, if any, antioxidants such as vitamin C have on chemotherapy.

Here's the thinking behind this one. Bendamustine is a chemo drug (an alkylating agent) that fights leukemia and lymphoma partly by creating cellular stress that pushes cancer cells to die. Vitamin C is an antioxidant, and in theory it could mop up some of that stress and make the chemo work a little less well. I want to be honest with you though: this concern is purely theoretical, based on how these things behave on paper, not on solid studies in real patients. We simply don't have proof that it helps or hurts.

Because the stakes with cancer treatment are high, I wouldn't add high-dose vitamin C supplements during bendamustine without running it by your oncologist first. Vitamin C from food and a normal multivitamin is generally not the worry here. Please loop in your cancer team before starting anything new.

What Is Bendamustine Injection Used For?

Brand names: Treanda, Belrapzo, Vivimusta

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
